InlandPolitics: ARMC director cuts deal with medical group he once labeled as “incompetent”

InlandPolitics.com has learned the Medical Director for San Bernardino County-owned Arrowhead Regional Medical Center (ARMC) has cut a deal with a Riverside County-based Anesthesiology Medical Group to replace its current provider.

There’s just two problems.

The first, no request for qualification (RFQ) or request for proposal (RFP) was ever initiated.

And the second, Medical Director Dr. Dev GnanaDev previously criticized the same medical group provider has being incompetent. Continue reading

Many people are asking why it appears San Bernardino County seems to be ignoring many of the problems occurring at its own Arrowhead Regional Medical Center (ARMC).

The answer that seems to be surfacing is political.

As in campaign contributions.

Sources tell InlandPolitcs that county politicians routinely request donations from medical service providers. Many of those contributions are handled through various medical groups. Continue reading

InlandPolitics: ARMC employees under pressure

This morning a meeting was held at San Bernardino County-owned Arrowhead Regional Medical Center (ARMC).

It was very interesting to say the least.

In politics, such a meeting is referred to as circling the wagons and is indicative that things aren’t going well for top management.

The meeting of the facility’s medical staff was conducted by Dr. David Lannum, Staff President.

What took place was absolute strong-arming, something that has become commonplace at the struggling medical center.

Sources tell inlandpolitics.com that hospital department heads were summoned to a conference room, where they were handed a sign up sheet to be circulated among staff and employees.

Signatures are being requested to show support for the quality of medical care, the staff, and most importantly GnanaDev.

InlandPolitics: ARMC staff attempting to mislead

This mornings article in the Inland Valley Daily Bulletin newspaper titled “Top doctors defend quality of care at ARMC” appears to have been written about a medical facility somewhere else.

Maybe in a “Banana Republic”.

While San Bernardino County and some of its doctors at county-owned Arrowhead Regional Medical Center (ARMC) try to mitigate and deflect allegations of misconduct and medical treatment gone haywire, more information continues to leak out of the facility that paints a grim picture.

News of five “sentinel events” involving the hospitals Obstetrics and Gynecology Department have surfaced.

iePolitics: More ARMC Cover Up

Sometimes I really wonder who is the worst lawyer, District Attorney Mike Ramos or County Counsel Ruth Stringer.  From Ramos we have learned that the Constitution of the United States is not applicable in San Bernardino County, at least not in connection with political enemies.   And County Counsel Ruth Stringer has taught us that copulation involving public resources is covered by the “deliberative process” and therefore not subject to review, not that we wanted to review the copulation, only the expenditures attached thereto.

Now we find our two legal beagles are at it again.  Well, not with each other we hope, but in consort of sorts, with those at Arrowhead Regional Medical Center (ARMC) who wish to cover for the misdeeds of members of the Board of Supervisors, the county’s Medical Director Dr. Dev GnanaDev,  ARMC Chief Executive Officer Patrick Petre, and a handful of doctors and other ARMC employees. Seeing the absolute in-your-face willingness of certain department heads and county officials to lie, cheat and cover up misdeeds, I have come to the conclusion that the Grand Jury needs to receive multiple complaints to spur them into action.  The complaint process is relatively easy.  The complaint form can be found here:  http://www.sbcounty.gov/grandjury/complaint.asp

InlandPolitics has learned from reliable sources that the Inland Counties Emergency Medical Agency (ICEMA) has been diverting critical trauma cases from San Bernardino County’s Arrowhead Regional Medical Center (ARMC) to Loma Linda University Medical Center (LLUMC).

iePolitics: ARMC Coverup

Evidence is surfacing that a major coverup is in progress at Arrowhead Regional Medical Center (ARMC).  Allegations surfaced months ago that San Bernardino County Board of Supervisors Second District Supervisor Paul Biane and Fifth District Supervisor Josie Gonzales received extensive free medical care at ARMC.  A complaint has been filed with the Fair Politicial Practices Commission (FPPC) alleging Form 700 violations due to the fact that these supervisors, along with former County Administrative Officer Mark Uffer, received free medical care, which constitutes a gift of public funds, and did not report said gifts as required by law on their Form 700s.

Today, iePolitics learned that ARMC Chief Executive Officer Patrick Petre at the behest of  ARMC Medical Director Dr. Dev Gnanadev, has ordered privacy statements  be signed and backdated by ARMC employees in regard to anything they witnessed in reference to members of the Board of Supervisors or other elected and appointed officials who may have been given free  medical care at the facility.  It appears that this process has been put into place to interfere with a criminal and/or Grand Jury investigation into practices at ARMC.

iePolitics has been contacted by employees who have been threatened with HIPAA violations if  they discuss the wrongdoing at ARMC.  It appears every effort is being made to sweep the current scandal under the rug. Continue reading

iePolitics: ARMC: Dr. Gnanadev and a Horrifying Revelation – Revised

Today I had the chance to meet directly with an Arrowhead Regional Medical Center (ARMC) doctor, who contacted iePolitics due to the articles we have been writing about the hospital.  The doctor provided iePolitics with a plethora of documentation to prove claims of mismanagement, inappropriate financial arrangements by ARMC Medical Director Dr. Dev Gnanadev, and a host of other accusations.  We will be discussing some of these issues in the days to come.

Out of all the discussion that took place this morning, one 60-second portion affected me more than anything else.  This doctor described a procedure that they have personally witnessed which is known to be occurring by Dr. Gnanadev for the purpose of bringing in more revenue for one of the groups he oversees, the Emergency Room doctors. iePolitics: ARMC – Time For Action

Yesterday we witnessed something at the San Bernardino County Board of Supervisors session that we don’t see often.  Subordinates of a sort spoke out against their administrator. Dr. John Steinmann, an orthopedic spine surgeon, and Dr. Keyvan Safdari, an anesthesiologist, both spoke of heavy handedness and retaliation by Arrowhead Regional Medical Center (ARMC) Medical Director, Dr. Dev Gnanadev.

Steinmann indicated to the members of the Board of Supervisors that patient care is suffering as a result of Gnanadev’s leadership.  As we have discussed here repeatedly, retaliation is the name of the game at ARMC, a practice likely initiated and certainly promulgated by former County Administrative Officer Mark Uffer.  And now we learn it is more true than we realized. iePolitics: Special Treatment at ARMC

When I read yesterday “And Comunale said not renewing the anesthesiology contract – which is held by one of Comunale’s relatives – is ‘just a vendetta on GnanaDev’s part’” in the San Bernardino Sun, I assumed the vendetta was against Comunale.  Well it turns out I was wrong.

As mentioned in the last Arrowhead Regional Medical Center (ARMC) article I wrote, San Bernardino County Supervisor Paul Biane had two MRI’s under anesthesia at the hospital.  As we have been reporting, board members frequently have procedures done without charting so that they do not have to pay for the procedures or co-pays.  And Biane expected the same in this instance.  But the anesthesiologist refused to complete the procedure without charting.  So Biane doesn’t want the contract renewed.  And Biane and Gnanadev cover for one another.  Are you listening Mike Ramos?

iePolitics: ARMC – Time for the Board of Supervisors to Act; Dr. Gnanadev Must Go

Earlier today a lengthy comment was added to a previous story about Arrowhead Regional Medical Center.  It came from someone identifying themselves as a Physician/Surgeon and read:

ARMC could be an excellent hospital. It is a very modern facility with dedicated nurses and staff who try very hard to do the best for their patients. The problem at Arrowhead is that the physicians who provide good patient care do not last. The reason for this is simple: they expect other physicians to provide equally good care and when they complain to Dr. Gnanadev their contract is cancelled. Continue reading

He blew the whistle on corruption in the County Assessor’s office—resulting in the removal and arrest of the Assessor and several of his associates.

John Doe No. 5 seems to have a selective memory problem.  Perhaps it is from all the alcohol or one too many late-night parking lot tête-à-tête, but something is skewed in Paul’s brain.  He seems to have forgotten he is a yet-to-be-charged possible defendant in San Bernardino County’s largest corruption scandal in history.  Mikey and Jerry refer to him as John Doe No. 5 or has he forgotten?

Perhaps  I’m being too harsh.  Maybe, I should give credit where credit is due.  In Biane’s quest to become the crime fighting hero of San Bernardino County, his actions did result in the arrest of several county officials, one of which may well be himself.  Now that is sacrafice for his constituents.  There, my dear voters, is a great reason to vote for Paul Biane.  He narc’d himself off. iePolitics: Dr. Gnanadev–Time To Resign Or Be Fired!–Revised

Well another “I told you so” for iePolitics.com and inlandpolitics.com.  We’ve been called liars and such over accusations about patient care, billing, and malpractice at Arrowhead Regional Medical Center (ARMC).  Now we learn that ARMC’s Medi-Cal and Medicare contracts are in jeopardy.

Both hospital director, Patrick Petre, and San Bernardino County Fifth District Supervisor, Josie Gonzales, attempt to downplay the significance of the actions taken by Centers for Medicare and Medicaid Services; however, a loss of these contracts would be substantial.  The hospital is required to provide care whether it gets paid for the services it provides or not and that includes care to Medicare and Medicaid patients. iePolitics: Patient Abuse and Malpractice at Arrowhead Regional Medical Center

Arrowhead Regional Medical Center (ARMC) continues to be the subject of numerous investigations and inquiries.  The hospital, once a shining star in our county, has lost its luster in wake of  accusations of mismanagement, malpractice, patient abuse, and fraud.  Much of the controversy involves the empire-building by former County Administrative Office Mark Uffer and ARMC Medical Director Dr. Dev Gnanadev.  Both men are known for their tyrannical control of every aspect of hospital operations, with Gnanadev even running roughshod over the always-despotic Uffer.

In their quest to gain recognition and respect for ARMC and, more importantly, themselves, both men plotted to contain any negative publicity about the hospital.  Employees who question practices or report misconduct are themselves subjected to threats of discipline, using the loss of  medical licenses due to phony accusations by hospital management as  a means of control. iePolitics: Possible Bombshells This Week

There is quite a potential for bombshells dropping this week in a number of ongoing corruption-related issues.

On Monday, we have the arraignment of former San Bernardino County Assessor Bill Postmus and former Chief of Staff Jim Erwin.  It is expected that the San Bernardino County District Attorney’s Office will turn over discovery for the case.   This may very well prove to be interesting.  First, the rumor that the District Attorney may ask for a gag order would be funny if it didn’t keep popping its ugly head up.  If true, it begs the question, “What is the District Attorney trying to hide?”  Well, that has birthed a few more rumors, the most prevalent of which is that among the discovery will be information showing that Second Supervisorial District Chief of Staff Matt Brown dropped a dime on his boss, Supervisor Paul Biane.  Rumors of Matt’s pending termination have been running rampant for a couple of weeks now. iePolitics: To Add to Jim’s Comments

I feel sometimes like we are beating a dead horse.  Unfortunately, that dead horse keeps coming back to life and rearing its ugly head.  There are several San Bernardino County department heads that have to go.  The county simply cannot afford to keep them on in these economic times.  Federal court and federal juries do not care how broke San Bernardino County may be when it awards judgments.  And Mr. Devereaux, you have some complete idiots (note HROs:  be sure to print this out to add it to “the list”) running the show who are going to ultimately cost taxpayers dearly.

At the top of the list is County Counsel Ruth Stringer.  How much longer are you going to tolerate her half-ass legal advice?  When you take an attitude of “we have 138 attorneys, so bring it on,” as did our former County Administrative Officer and our current County Counsel has, you’re assuming you can outlast us all.  Not all of us go away easily or cheaply. iePolitics: More on ARMC

I got busy and didn’t follow up on a couple of comments posted to one of the entries regarding patient abuse at Arrowhead Regional Medical Center.  Here are the comments:

The state came down one day and talked with one of the patients who was being kept in wrist to waist restraints 24 hours a day. This patient began to have skin (stage 1) break down on thier wrist and ankles. He began to behave like a caged animal, after three months. The patient was not being bathed daily, and was being mistreated daily. All this was being done to this patient at the demand of Paula Martin. The nursing staff was upset about this, but where told if they refused to do what they were told they would be fired. When the state came, Kilie Torres told the patient to tell them that he was happy being tied down like this.Lets take in account that this patients mental status was well below a normal 20 years. “Of course he would tell the state that, She came a gave him candy, right before the interview”.

As far a Paula Martin is concerned, if she stayed off the “ETOH”, she could have done a better job. When did Arrowhead start employing alcoholic’s to run the behavioral health? As far as Natilie, she stays so drugged induced,how could she even begin to do a professional job. Now Valerie Flory, who in the world would want someone so unethical, evil,lacking morals to care for their love one as a nurse. This women, promoted a employee in her department, only to place him back on probation, after years of employement, only to set him up to fire him.”She has that mans blood on her hands” she should be ashamed. Paula Martin catered to this women and allowed her to go to school, and work one day, only because Valerie knew to much about the malvolent things going on in administration.

The issues raised by this blogger are exactly what I’ve been talking about for months.  These events are common knowledge among behavioral health staff but ARMC brass continue to cover up the patient abuse.  Something needs to be done!



iePolitics: The ARMC Mess — Part 4

I promised this quick entry a while back but got sidetracked with life.  I cannot say this situation is exclusive to Arrowhead Regional Medical Center (ARMC) as staff there who have worked in other hospitals say that it seems to be commonplace everywhere they have worked.  I’m not sure if it can be chalked up to the stress of working in life and death situations or just the mentality of those that go into the medical profession, but it definitely is a very significant part of the culture at ARMC.

Of course, I’m talking about sex, sex and more sex.  Sex on the job, sex in the parking lot, sex in the broom closet, sex in the patients’ bathrooms, sex in the staff lounge, sex anywhere there is not a camera and probably sex where there are cameras.   I wonder if they ever do it in the morgue?   Everybody is screwing everybody in that place and some of it occurs on county time. iePolitics: The ARMC Mess — Part 3

As we find out that San Bernardino County’s budget situation is far worst than we were led to believe by former County Administrative Officer Mark Uffer, the situation at Arrowhead Regional Medical Center (ARMC) become even more disheartening.

Someone needs to correct me if I’m wrong, but as I understand it, ARMC is operated with an Enterprise Fund.  This means that profits, if that is the right term, are turned over to the county’s general fund.  Therefore, it is a good thing when ARMC is able to collect reimbursements from Medi-cal, MediCare, insurers, the state of California (i.e. state prisoners), and the federal government (illegal alien health care).

Unfortunately, the situation with billing at ARMC is an absolute mess.  I know.  I was involved in attempting to clean up one such mess as part of my assignment at ARMC.  What I personally witnessed was over two years worth of medical billings that were never processed.  So what does this mean? iePolitics: Cleaning Up Corruption or Political Assassination

As I was writing the articles today about Arrowhead Regional Medical Center (ARMC) and Mark Uffer, the thought that came to mind was how much of a bigger story the ARMC situation is than the Assessor’s Office scandal or even the Colonies settlement.  It not even just bigger from the standpoint of corruption, but also from the standpoint of a scandal that really would resonate with voters.

If one were to ask the average voter on the street about the Assessor’s Office or Colonies, likely he or she would not even know what one is talking about.  Voters are too busy to care about shenanigans in the Assessor’s Office and don’t care to take the time to learn about Colonies. iePolitics: ARMC Doctors Violate Patients’ Civil Rights

5150. When any person, as a result of mental disorder, is a danger to others, or to himself or herself, or gravely disabled, a peace officer, member of the attending staff, as defined by regulation, of an evaluation facility designated by the county, designated members of a mobile crisis team provided by Section 5651.7, or other professional person designated by the county may, upon probable cause, take, or cause to be taken, the person into custody and place him or her in a facility designated by the county and approved by the State Department of Mental Health as a facility for 72-hour treatment and evaluation.

5151. If the facility for 72-hour treatment and evaluation admits the person, it may detain him or her for evaluation and treatment for a period not to exceed 72 hours.

Arrowhead Regional Medical Center (ARMC) has a long-standing policy in effect that they say gives them the right to detain patients longer, sometimes much longer, than 72 hours as authorized by Welfare and Institution Code sections 5150-5151.  San Bernardino County’s crack legal beagles has interpreted these code sections to mean that the 72 hours does not start until the patient is admitted.  That seems like a minor problem unless one understands the “admission process.”

ARMC’s admission process often takes 24-48 hours.  When patients are brought in to ARMC for evaluation, they go through what most would think is the evaluation process.  They are searched; asked to change into hospital garments; asked about medications, etc.; urine and blood samples are taken; a medical, family and social history is taken; an EKG is performed; and they are seen by a RN, an MD, and a psychiatrist for evaluations.  However, none of this is considered part of the “72-hour evaluation.”  It is not until after all of this is done and the patient is transferred from a room in the Triage Unit to a room in a regular unit does the 72 hours begin.

I am going to be writing several Arrowhead Regional Medical Center (ARMC) pieces today and in the days to come and thought I would take a moment to make one clarification.  I suspect, based on previous articles, that some readers are under the impression that I have an issue with Patrick Petre. I do not.

Is anyone besides me laughing hysterically about this turn of events?  I mean we sort of expected it but now that it finally hit we have to wonder what the heck he is thinking.  Well I’m sure we know what he is thinking.

Mark Uffer is the most hated county employee in county history.  When his termination was announced, there were reports that employees in some offices stood and cheered at the news.  He stands no chance of finding another job so he is looking for a quick buck. Continue reading

iePolitics: Drug Supplier to the Board of Supervisors

We have all read the stories about the “doctors” to the stars.  You know the ones:  doctors who would give the likes of Marilyn Monroe, Elvis Presley, and Michael Jackson their prescriptions of choice whether it was medically advisable or not.  Well it seems that our “VIPs” from the county of San Bernardino have their own doctor to the “stars” . . . umm VIPs . . . um Board of Supervisors.

VIP treatment for San Bernardino County members of the Board of Supervisors is provided by one of Arrowhead Regional Medical Center’s doctors, a Dr. James Evans.  It seems that even if a board member or other politico has an obvious substance abuse problem, or it should be obvious to someone in the medical profession, they can still get prescription pain medication without a doctor’s visit.  Another fine use of taxpayer money.

 

iePolitics: FPPC Complaints Filed

FPPC complaints have been filed against San Bernardino County Supervisors Josie Gonzales and Paul Biane as well as former County Administrative Officer Mark Uffer.  The complaints all allege that these three individuals, along with countless other politicians and department managers, have been the recipients of free medical services at the county’s hospital, Arrowhead Regional Medical Center (ARMC), and have failed to report the gifts on their Statements of Economic Interests (Form 700).

VIPs, as they are known at the county-run hospital, routinely have medical treatment arranged for them free of charge at the ARMC at taxpayer expense.  Often, in order to hide the free treatment, no computer records or medical charts are created.  VIPs, if being treated in the emergency room, are moved ahead of even critically ill patients. I had a different aspect of the Arrowhead Regional Medical Center (ARMC) mess planned for this installment but a new twist occurred before I could get internet access long enough to write it.  I’ve written about falsification of records and patient abuse before but couldn’t name names because I might put others at risk.  However, three of the problem employees apparently are no longer in positions of power.

First and foremost is Paula Martin, the former manager of the  Behavioral Health Unit.  She saw the writing on the wall and has left the employ of San Bernardino County.  I feel sorry for the new facility she will run into the ground. Continue reading

From the Sentinel . . .

Investigators Looking Into Arrowhead Regional’s Operations

Allegations of malpractice, licensing shortcomings, operational and managerial miscues and substandard accounting practices at the county hospital are the focus of separate inquiries by federal and state agencies, the Sentinel has learned.

The Sentinel was unable to ascertain whether a rumored survey of the hospital’s operations by auditors working at the behest of the board of supervisors is actually underway.

The issues being looked into include current practices as well as those extending back nearly a decade at the Colton-based facility, which is known as the Arrowhead Regional Medical Center.

The sharpest focus is that on the administrative action of Dev GnanaDev, who is both the current medical director of Arrowhead Regional as well as its head of surgery. GnanaDev also happens to be the president of the California Medical Association.
Some of the suspected problems at the hospital originated at a level well below that of GnanaDev and involved several instances of medical malpractice and faulty diagnoses. Others have been attributed to GnanaDev directly.

One known case,  the Sentinel is informed, elicited the interest of federal investigators and involved a thirteen year old girl who had suffered a dislocated neck. Physicians and staff failed to adequately recognize or diagnose the condition, resulting in the child becoming permanently paralyzed. The medical staff further stands accused of having lied to the parents about the situation.

Since you asked, here is contact information for several local Federal Bureau of Investigation offices.  FYI, numerous county officials have contacted the Riverside FBI office, interviewed with them, and have requested investigations into wrongdoing in San Bernardino County.  So far, nothing concrete has happened.  Because so many have already contacted that particular office, I would tend to think they could also stand to hear from some private citizens.

The situation at Arrowhead Regional Medical Center is more serious than even I realized.  I have not been writing about it because of the type of investigation that is going on.  This investigation is not being completed by the county or standard state agencies that normally investigate the hospital.  I will update as information becomes available.  I do expect to see at least one termination at a very high level and there is a potential for criminal charges being brought. iePolitics: Arrowhead Regional Medical Center

I know we have a number of Arrowhead Regional Medical Center (ARMC) staff, including some of its doctors, reading this blog.  I need some advice.

I wrote a short while back how I was treated by Dr. Pham.  I contacted Patient Rights at ARMC and was assured my complaint was being elevated.  I was supposed to hear back by January 11 and still haven’t heard a word.

iePolitics: Assertive Rights

Many of my friends have been through the Intensive Outpatient Program (IOP) at Kaiser and have highly recommended it to me.  If it weren’t for county employees, those groups would be empty.  I tried to go some months ago, but my supervisor at Arrowhead Regional Medical Center came unglued at the thought.  More recently, my doctor all but insisted on it.

I know how many county employees suffer from depression and I can’t recommend the group enough.  If you have a supervisor like mine, ask to see Dr. Belin.  Mark Uffer ended her county career after 25 years, and as she keeps telling me, there are a lot better places to work than San Bernardino County.  She understands what we are going through and will tell the county to “pound sand” on your behalf if she thinks you will benefit from the program.

iePolitics: The Little Things In Life

I know we still have a lot of former San Bernardino County Jobs and Employment Services Department Employment Services Specialists reading the blog. How many of you remember the days before TAD decimated our department when we went through programs to help make us better employees rather than destroy our ability to get our job done?   Anyone remember the poverty training we had to go through?  Somewhere I still have the book describing the survival skills necessary to be living in a lower-class situation, a middle-class situation, and an upper-class situation.

Do remember the questions we had to answer?

  • Do you know how to get the executive membership at the country club?
  • Do you know how to keep your laundry from being stolen at the laundromat?
  • Do you know how to pick a daycare center?
  • Do you know the time of day stores deposit their expired food in the dumpster?
  • Do you know how to get your child into the most exclusive private schools?
  • Do you know where the food banks are located and when they give out food?
  • Do you know how to hire a maid?
  • Can you tell the difference between silk, linen and cotton and how to care for each?
  • Do you know how to get your children to fall asleep when they are still hungry?

iePolitics: Arrowhead Regional Medical Center Update

I have finally had a chance to talk to a few of my sources.  Additional information is being brought forward regarding falsification of records, including commitment records for behavioral health patients.  It seems patients are being kept past their 72-hour commitments based on forged records by nurses rather than by actual doctors orders.   More to come . . .

iePolitics: Stories We’re Working On

At the top of my priority list right now is revealing the corruption within San Bernardino County’s Program Integrity Division, better known as the Welfare Fraud Unit, headed by Virginia Lugo. Welfare Fraud is the lowest of priorities in this county even though both internal and external fraud runs rampant.

I was discussing the issue with former San Bernardino County Chairman of the Board of Supervisors Bill Postmus this morning. It seems the two administrations since his have reduced the number of fraud investigators by over 75 percent in the past few years. This is just another example of how out of touch our current Board of Supervisors is with the public they have been elected to serve. Continue reading
